RESPONSIBILITIES:
 
  • Oversight of entire program, including planning, implementation, and monitoring to ensure high-quality services aimed at keeping older adults healthy, well, connected and full participants in their community
  • Conduct regular team meetings and work closely with team members to develop programs, service delivery and projects
  • Monitor staff performance levels with respect to contract requirements, workloads, and program quality
  • Develop and implement outreach strategies to reach all residents, including those severely isolated
  • Timely reporting to all funders and partners; regular review of priorities and policies to inform program excellence
  • Prepare program annual budget and monitor expenses in conjunction with agency finance director and CEO Collaborate with community organizations, neighborhood businesses, and local officials to support Neighborhood  residents
  • Strategize and implement ways to increase financial investment, partnerships and community support
  • Facilitate monthly Residents’ Council Meetings; recruit new members as necessary
  • Represent Neighborhood  at public meetings and related events
  • Draft, edit, and review written deliverables such as print and electronic newsletters, brochures, social media posts and website content in collaboration with Marketing staff
  • Support volunteer and intern recruitment and supervision
  • Other duties as assigned by agency CEO

QUALIFICATIONS:

  • Bachelor’s Degree in social work or related field required; Master’s Degree in Public Administration, Public Health, Nonprofit Management or Social Work preferred
  • At least three to five (3 – 5) years’ experience working with older adults
  • Supervisory experience strongly preferred
  • Excellent organizational skills
  • Must be computer literate and comfortable utilizing various technological platforms to facilitate and participate in virtual programming and service delivery
  • Excellent verbal and written communication skills. Creative, innovative and flexible
  • Must have valid driver’s license, personal vehicle and be willing to travel throughout Capital Region as needed

MyFamsc is seeking a part-time Registered Nurse for the Neighborhood 
 
RESPONSIBILITIES:
  • Conduct blood pressure screenings and provide opportunities for residents to discuss their health concerns at regularly scheduled times at various locations within the Neighborhood .
  • Provide in-home visits to Neighborhood  residents as needed to monitor acute and chronic health conditions.
  • Maintain accurate records of service delivered to Neighborhood  residents; collect related statistical information.
  • Develop individual care plans when necessary and provide related healthcare case management.
  • Communicate with agency colleagues and resident family members, as appropriate, regarding the health status of Neighborhood  resident and additional services needed.
  • Provide regular public health related programs to Neighborhood  residents and participate in other Neighborhood  programs, including immunization clinics, as needed.
  • Create partnerships with local physicians and health care programs to identify homebound seniors and provide services and supports to these individuals.
  • Stay current on research and trends related to health care for seniors, and assist in the development of new programs, in response to community needs and requests.
  • Maintain client confidentiality in accordance with agency policy and federal law.
  • Understand and practice agency mission and values.

QUALIFICATIONS:

  • Current NYS Registered Nurse license
  • Experience in working with seniors; possess an understanding of the relationship between good physical, mental and emotional health and successful aging-in-place.
  • Excellent communication and presentation skills; ability to collaborate well with team members, outside agencies and service providers
  • Proficient with Microsoft Office 365
  • Valid New York State driver’s license with reliable, personal transportation. Ability to travel throughout the greater Capital Region, as needed.

RESPONSIBILITES
 
  • Administer all requests for MyFamsc services, including food security, housing security, financial assistance, transportation, mental health, physical health, OT/PT, healthcare advocacy and support
  • Responsible for intakes, agency assessments, consents, releases, internal referrals and documentation
  • Complete and track all applications to outside agencies and programs and referrals
  • Coordinate with team members; participate in Clinical Team Meetings and general staff meetings
  • Update job knowledge by participating in educational and training opportunities, balancing work requirements with learning opportunities
  • Improve treatment outcomes by evaluating and re-designing processes, implementing and revising policies and procedures
  • Prepare reports by collecting, analyzing, and summarizing client outcome data and trends, compiling statistics for grants and program evaluations
  • Perform other appropriate duties as requested by Director of Clinical Services, Director of Neighborhood or CEO

QUALIFICATIONS:

  • Minimum of three years of relevant experience; Master’s degree within the field of Mental Health or Social Work preferred; active New York State License: LMHC, LCSW, or LCSW-R preferred
  • Experience working with older adults
  • Excellent communication and presentation skills
  • Ability to collaborate well with team members, outside agencies and service providers
  • Proficient with Microsoft Office 365
